Unfortunately, we do NOT take Medicare or AHCCS.
If you are using your in-network health insurance benefits, click the Make an Appointment button on the home screen to select an available appointment date and time. New clients should select a 60 minute "New Patient Appointment", and existing clients should select a 30 minute "Follow-Up Appointment". We will verify your insurance, contact you to confirm your appointment, and send you any required paperwork that can be completed electronically. We utilize Headway and Alma as our billing partner and any copays or coinsurance payments will be processed through them.
In health insurance, a deductible is the amount of money that you are required to pay out-of-pocket for covered healthcare services before your insurance plan starts to pay for those services. Essentially, it's the initial portion of your medical expenses that you must pay yourself before your insurance coverage kicks in.
For example, let's say you have a health insurance plan with a $1,000 deductible. If you incur medical expenses covered by your insurance, you would need to pay the first $1,000 of those expenses yourself. After you've met your deductible, your insurance plan would then begin to cover a portion of the remaining costs, such as through copayments or coinsurance, depending on the specifics of your plan.
It's important to note that not all healthcare services may count toward your deductible. Some services, such as preventive care (like routine check-ups, vaccinations, and screenings), may be covered by your insurance plan without having to meet your deductible first.
Once you've met your deductible for the year, your insurance coverage typically shifts to a coinsurance or copayment structure, where you share the costs of covered services with your insurance company until you reach your out-of-pocket maximum for the year. After reaching your out-of-pocket maximum, your insurance plan typically covers 100% of covered expenses for the remainder of the plan year.
Our cash pay price is $225 for new patient appointments, $275 for ADHD evaluations, and $125 for 30-minute follow-up appointments and $200 for 60-minute follow-ups. Payment is expected at time of service.
When we provide a service, we count on your payment to compensate your provider for their time. Legally, you're obligated to pay copays once your insurance contributes towards your visit. Failure to make payments may result in your account being forwarded to a collections agency, and an additional fee of $200 will be applied.
Also remember, we have families to take care of and student loans to pay. While we would love to provide free service, it just isn't possible.
Yes! We offer in person visits in the Mesa/Gilbert area and via telehealth from the comfort of your couch! Initial visits with clients under the age of 12 must be done in person although follow-ups can be done via telehealth. We often get asked if virtual or in-person is better. There is no right answer! Whichever you choose, you will get the same personalized service!
Prescription of controlled substances, including stimulants, benzodiazepines, and suboxone will require a signed consent by the patient acknowledging the following:
Our approach to benzodiazepines, such as Xanax, Klonopin, and Ativan is cautious, recognizing their benefits for short-term use in certain conditions while also acknowledging their addiction potential and other risks. Long-term or high-dose use of benzodiazepines is not supported by the FDA or evidence-based research, and benzodiazepines will not be prescribed in these ways at Hope 3:16 Psychiatric Care, LLC
The short answer: Yes. The long answer: Stimulant medications are prescribed with stringent criteria, focusing on extended-release formulations to mitigate risk of misuse or dependence. Immediate-release stimulants may not be prescribed by your provider. Thorough assessments are conducted to ensure appropriate use of stimulants, and this may include requirements for blood pressure monitoring and routine EKG’s
Realistically, being on-call 24/7 is not feasible. Like you, your provider has a family and other obligations. If you need to speak with your provider, scheduling an appointment is the best way to address any concerns or changes.
We do not provide crisis services. In case of an emergency, please call 911, contact the crisis hotline at 988, visit your nearest urgent care center, Mind 24/7, or go to the emergency room.
Yes! We will need to discuss in length during an evaluation about your emotional and psychological needs and find out more about the animal(s) in question. We do not prescribe service animals. Please understand the difference prior to making a request. We also do not assume responsibility or liability for the behavior, actions, or costs associated with the ESA nor the behavior of the animal. We do charge for paperwork.
We do fill out FMLA and short-term disability paperwork, however we do not complete disability (SSA or SSDI) paperwork. We do charge for paperwork as this is completed outside of appointment time and is not covered by insurance
